With the start of the 2011-2012 school year every student will need to have two vaccinations
for chickenpox or will need to show documentation from a parent/guardian
that the child has had this disease.  If your child has had chickenpox, you can
fulfill the new law requirements by completing the Parent Verification Form.  
Please download this form and return it to your child’s school office.
