Notice of Availability of IRS Forms 1095-B and 1095-C
The school provides IRS Form 1095-C Employer-Provided Health Insurance Offer and Coverage to employees upon request. These forms include information about health coverage offered to you, if any, during the prior calendar year. You may
obtain a copy of your Form 1095-C upon request. Requests may be submitted using any of the following methods: email, written request via mail, or telephone request to the business office.

Upon receipt of a request, the school will furnish the requested form within 30 days of the date the request is received. The form may be furnished electronically if the requesting individual affirmatively consents to electronic delivery. IRS Form 1095-B, Health Coverage, is prepared and submitted to the IRS by Blue Cross and Blue Shield of Nebraska. If an employee would like to request a copy of the Form 1095-B that BCBSNE submits to the IRS, the employee should contact BCBSNE Customer Service at the telephone number listed on the back of the employee’s health insurance identification card. Upon request, BCBSNE will provid a copy of the Form 1095-B directly to the individual.
